Referral Pathway

We offer a clear, fast-track referral process for professionals. Submit a referral form, and our team will review compatibility, conduct an assessment, and confirm availability within agreed timeframes.

Client Groups

We accommodate adults requiring low to medium support, including those transitioning from residential care, leaving prison, or experiencing homelessness who need a structured stepping stone to independence.

Support Model

Our model is strength-based and person-centred. We focus on harm reduction, positive risk-taking, and empowering residents to achieve their goals through structured support plans.

Risk Management

We conduct initial risk assessments prior to placement and ongoing dynamic risk management. We work closely with referrers to ensure environments remain safe for both residents and the local community.
